OK I stripped the top half of code down to `phplinks_track` and uploaded, captcha works great now, but I still got another SQL upload error and don't know if it is effecting anything or not at this point:

I get this error under /admin page when trying to validate the submitted site I put through the 'Add A Site' page:


Invalid query: Unknown column 'phplinks_temp.track' in 'field list'

Since you only have sample entries, it is probably best to use PHPMyAdmin to drop (as in delete) the phplinks_temp and phplinks_links tables then use the CREATE TABLE queries for phplinks_temp and phplinks_links tables to recreate them.

As for the query you got the error on, that makes a sample entry of Dew-Code.com along with a few others, and while appreciated isn't necessary for the script to run, but it does indicate that the tables mentioned above need to be upgraded.
